What is manual therapy?
Your physiotherapist uses graded joint mobilisations, soft-tissue release and stretches to reduce pain and restore movement, then gives you exercises to keep the gains.
Techniques include rhythmic joint gliding (mobilisation), sustained stretches, soft-tissue and trigger-point release. These calm sensitive tissues and the nervous system, reduce protective muscle guarding and improve joint movement. Guidelines recommend manual therapy combined with exercise rather than alone.
Also called: Joint mobilisation, Soft tissue mobilisation
What it can help with
When manual therapy is not suitable
Your physio screens for these at the first visit and refers you to a doctor if needed.
- Recent fracture or unhealed surgery at the area — Only once your surgeon clears it.
- Severe osteoporosis — Techniques are modified to be very gentle.
- Red-flag symptoms — Night pain, unexplained weight loss, numbness in the saddle area or loss of bladder/bowel control need a doctor first.
- Inflammatory flare or infection — Treatment waits until it settles.
Who manual therapy can help
Your physio decides after the assessment. It is one part of a plan, never the whole plan.

Questions people ask
Is it the same as a massage?
No. It is clinical treatment chosen after an assessment and combined with exercise.
Will you “crack” my back?
We mostly use gentle mobilisations. High-velocity manipulation is only used if suitable and with your consent.
How soon will I feel better?
Many people move more easily after the first session; lasting change comes with regular exercise.
